If you would like to view a copy of your report, you can use this portion of our web site to find out how to request a copy of your report and how to initiate an investigation of disputed records. If you have recently applied for a product or service from one of our subscribers, their decision may have been based, in part, on the information provided to them by Teletrack . You are entitled to receive a copy of that same information free of charge. Please complete a consumer request form, which can be downloaded here. Return the form to our office:

The form will not be considered complete unless you have signed your name in the designated place and included a copy of your Driver's License.

Please note that we cannot inform you of the specific reasons why you were denied. We can only supply you with the same information given to the lender at the time of your application.

Request an Investigation
Upon reviewing a copy of your report if you find information recorded on the report that you wish to dispute, you may request an investigation by calling 1.877.309.5226. Teletrack conducts investigations free of charge.
